Medical certificate for those employees of Central and State Government institutions and Public Sector Undertakings and others under treatment from an institution of the department of ISM will be provided. The certificate will be provided by the Medical Officer who is managing the treatment. Certificates will not be provided to those who are not under the treatment from the institutions of the department. Patients should visit the institution in person with any photo identity card like aadhar, electoral Id card, driving licence etc. Employees may keep the official id cards also. The person has to be physically present before the Medical Officer with all the relevant documents and affix the signature / thumb impression in the place provided in the certificate.
