Fitness Certificate will be provided by the Medical Officer(s) of the department for proving the physical and mental fitness of a person (employee in a Governmental or non-Governmental institution) for resuming his / her duties after a period of leave on medical grounds. The person has to be physically present before the Medical Officer with all the relevant documents including the Medical Certificate based on which leave was availed and affix the signature / thumb impression in the place provided in the certificate.
